N i have mailed this form today after filling it properly and got FINI619 instructor reply just after 20 minutes which is mentioned here:

Dear Student,

 

Your request for the issuance of Internship Reference Letter (IRL) has been forwarded to the concerned department. Your reference letter will be issued soon. Your internship MUST be approved by the Head Office of the organization and/or must be according to the policy of the organization in this regard. Make sure the Internship Completion Certificate shall only be acceptable if issued by the designated authority.
