bro  send the original ICC and evaluation form to the university by TCS on the following
 
address;Instructor Fini619, Department of Management Sciences, Virtual University of Pakistan, M.A. Jinnah campus, Defence road off Raiwind road, Lahore. UAN:(042 111-880-880)
and also u must paste the scane copy of ICC and the letter of undertaken in the internsip report
the evaluation is sent by your organization to the university,however if university refuse to do u then u can sent the ICC and the evaluation form in one envolpe to the above montined address. if u any more quires then mail me bro m b there for ur help 
 
i also attach the Letter of undertaken please find for the attachment
